Canine Massage

Before I can massage your pet I will need to obtain Veterinary Consent. Please download the form and have your Veternarian sign it and bring it or email it to me before your dogs first massage appointment.

*Pet Massage is not, nor is it intended to be a substitute for traditional veterinary care, if necessary; and upon your request we will be happy to work with your preferred vet to discuss your pet’s special needs.

Boarding Info Form First Dog

Drop off can be caotic. Please print and fill out the Boarding Info Form and bring it with you. If you do not have a printer you can fill the forms out online and email them to me info@dogoasis.net. If you dont have time or can't do either DONT WORRY. I have plenty of blank forms here for you. If you have any questions about the forms please contact me. This form is TWO Pages. Waiver on back must be signed and dated. One waiver for all dogs. Please list all dogs names on waiver.
